Abstract
This was a prospective, randomized, open-label, and placebo-controlled study to assess the impact of the co-administration of tibolone with, preoperative gonadotropin-releasing hormone (GnRH) analogue treatment on 121 premenopausal women, who had laparoscopic, preoperative myometectomy of the uterine leiomyoma against placebo. The participants were to be exposed to the leuprolide acetate with tibolone and iron (Group A), leuprolide acetate with placebo and iron (Group B), and iron alone (Group C). Both GnRH analogue-treated groups showed a significant decrease in uterine and leiomyoma volume, an improvement in hematologic parameter and a change of myoma-related symptoms prior to surgery. Without impairing the reduction of the fibroid, tibolone was effective in reducing hypoestrogenic vasomotor symptoms. Group A and B had better operative outcomes as indicated by surgical time and intraoperative blood loss, as compared to iron alone. These results demonstrate that preoperative GnRH analogue treatment is beneficial to minimize the size and severity of fibroid mass and symptomatology, and tibolone is a safe and well-tolerated alternative to improve patient adherence and comfort.
